The organisation of football in Lesotho is structured according to a hierarchical league system, allowing clubs to ascend or descend based on their performances. This structure is divided into several levels, ranging from professional football to amateur football.

At the top of the hierarchy is the Lesotho Premier League, which is the highest level of football in Lesotho. It comprises 14 professional clubs that compete for the title of Lesotho champion. The poorly ranked clubs at the end of the season are relegated to the A-Division, which also includes several clubs divided into two groups.

The A-Division, the second level of football in Lesotho, is subdivided into two groups: the North group and the South group. The best-ranked clubs from each group are promoted to the Lesotho Premier League, while the poorly ranked clubs are relegated to the B-Division.

The B-Division, the third level of football in Lesotho, is also subdivided into several regional groups. It includes amateur clubs as well as reserve teams from professional clubs. The best-ranked clubs from each group are promoted to the A-Division.
